MDA Students - Return from Leave of Absence Request Form

Submit this form to request a return from a Leave of Absence.
It is important that you review the letter that approved your leave stating the requirements established at the time of your Leave Approval.
Deadlines:
Summer semester return request deadline - Feb 1
Fall semester return request deadline - June 1
Spring semester return deadline - October 1
These deadlines are established to best ensure students are registered for class and receive tuition invoices.
